CANCELLATIONS: Please allow at least 24 hour notice for any cancellations or rescheduling.   Cancellations made within 24 hours prior to the appointment will be charged a late cancellation fee of $45.

Payment is requested at the end of each appointment.  Debit, credit, and check are accepted, as well as FSA and HSA (in accordance with their requirements).
